衡山基金會於台中成立「衡山社區復健中心」協助精神障礙朋友生活復健，重返社會。長程願景是籌建「衡山之家」結合綠自然照護，協助更多青年精神障礙朋友復健療育。提供「行善團」服務，協助身心障礙家庭居家修繕，幫助弱勢族群走出生活困境，擁有安全舒適的避風港；另有「食物銀行」提供弱勢家庭生活上所需物資。
